Incident Reporter™ v2.0 now meets ICAO, Transport Canada and FAA requirements for risk assessment, including assessment of substitute risk and acceptance of residual risk by appropriate risk-decision authority.
Controls have also been added that enable administrators to “lock” Corrective Action Plans (CAPs) after they have been accepted, and that prohibit closing of records unless all risk controls, corrective actions, follow-up audits and hazard/CAP communications have been satisfactorily completed.
Incident Reporter’s™ functional layout and intuitive interface permit fast and easy navigation. Nine “tabs” are used to record incident, accident or event data, including detailed descriptions and photos. Most fields are either a pop-up menu or a button, so typing is minimal for you two-fingered typists. The blue navigation buttons near the top are speed buttons to quickly jump between screens.
Enter risk statements and Relative Calculated Risk, a formula that rates—on a scale of 0 to 100—the risk of an accident or event happening or reoccurring based on the exposure, likelihood, and severity of the risk.
Develop multi-faceted Corrective Action Plans that permit assignment of risk controls and corrective actions to specific department managers or supervisors. Use administrative authority to “lock” the CAP after it has been accepted, while managers continue to be able to enter their corrective action, monitoring and hazard/CAP communication activities.
