Chapter 01: Program Control
OAG hosts your Safety Management Program on a secure Microsoft SharePoint site for secure 24/7 access, anywhere an internet connection is available. The current revision of your SMP and Forms Catalog are “locked” for access & printing by your employees. FAA personnel may also access, comment and watch you edit in real time. Using secure usernames and passwords, one or more of your managers may edit the program and forms using MS WORD. OAG’s simplified revision procedures and detailed instructions make the revision process easy.
Chapter 02: Safety Management Plan
SMS objectives and recommended specific safety goals are provided. Your Company’s positive safety culture is defined, including your Safety Management Policy Statement. Working together, OAG customizes the program to meet your desired policy, objectives and goals.
Chapter 03: Organization
This chapter is customized to meet the size and scope of your organization and management resources, based on information you provide in the OAG SMS Information Document (SMSID). Includes a description of all support services provided by OAG.
Chapter 04: Training & Qualification
The Training Program specifies four categories / courses of training: SMS Training for Personnel, SMS Training for Managers, SMS Training for your Safety Officer & Auditors, and Vendor Employee SMS Training. Objectives, prerequisites, courseware and completion standards are defined, and all training is conducted on-line through rich, interactive training modules. Sample the training by taking OAG’s SMS Short-course. This informative course explains the “hows and whys” of SMS, and the many benefits it can provide. It is offered free of charge to the aviation industry, in the interest of aviation safety.
Chapter 05: Confidential Reporting System
Chapter 5’s Confidential Reporting System was developed in accordance with FAA Advisory Circular AC 120-66B Aviation Safety Action Programs (ASAP), and is utilized by all personnel and vendor employees for the reporting of hazards, irregularities, errors, violations, near-accidents, “close calls” and other safety concerns. This non-punitive system encourages employees to report these events without fear of retribution. On-line reporting capability enables company personnel and vendor employees to submit certain reports anonymously with guaranteed de-identification of data. Includes a Voluntary Disclosure Reporting Program with web-based VDRP in accordance with AC 00-58A.
Chapter 06: Risk Management Program
Risk management is at the heart of SMS. Integrate your existing trend monitoring, accident prevention and loss prevention programs, or modify and adopt OAG programs to control risks. Data is taken from the Confidential Reporting System, Risk Control Worksheets, Audit Findings, Accident / Incident Investigation Reports and external sources. Hazards are either eliminated or risks mitigated through Risk Control / Corrective Action Plans (RC/CAPs). Using the RC/CAP form, risk controls and/or corrective actions are documented, prioritized and implemented, areas of responsibility are assigned and follow-up audits performed. Hazards, risk controls and corrective actions are then communicated to all employees and vendors as lessons learned.
This chapter also includes Operational Risk Management (ORM) techniques which enable personnel at all levels within your organization to make sound decisions. Applied ORM results in fewer operational disruptions and increased productivity. The ORM process also enables managers to identify hazards and develop risk controls during design and planning stages for growth, new equipment, facilities, acquisitions, etc.
Chapter 07: Internal Evaluation Program
Incorporate your current IEP into your SMS or modify and adopt the OAG IEP, which confirms both regulatory compliance through Departmental Audits and measures process outputs through Performance Assessments. Audit findings are channeled into the Risk Management Program for corrective action. Developed in accordance with AC 120-59A, Chapter 7’s IEP is a complimentary and necessary component of a quality Safety Management System.
Chapter 08: SMS Forms
A complete set of forms for documenting and managing your SMS are provided in Chapter 8. Use the OAG forms or incorporate your current irregularity report, error report, injury report, etc. Forms are hosted on your secure SharePoint site for easy access by personnel and electronic submission to your Safety Officer.
Chapter 09: Process Measurement
Process measurement ensures the quality of your SMS, and is a necessary component of modern Safety Management Systems. Process measurement enables the identification of opportunities for improvement.
Chapter 10: Accident / Incident Investigation
This chapter covers modern investigation methods & techniques for identifying systemic deficiencies (latent conditions) that may have contributed to an accident. Near-accidents, “close-calls” and other safety-related events are also investigated, with results being channeled into the Risk Management Program for corrective action. The roles, responsibilities & relationships of NTSB, FAA, company representatives and other participants during accident investigation are also addressed. Managers who complete SMS Online Training will be able to apply the methods and techniques described herein to effectively investigate company accidents, incidents, occurrences, near-accidents and other safety-related events.
Chapter 11: Emergency Response Plan
The OAG Emergency Response Plan is comprehensive and includes procedures and checklists for aircraft accidents & incidents, ground incidents, injury or death, overdue or missing aircraft, hijacking, ground facility and aircraft bomb threats, suspicious items / vehicles and explosions, hazardous / radioactive material incidents and simulation exercises. A Family Assistance Plan and SOS procedures (requires separate agreement with International SOS) are also included. Adopt OAG procedures or incorporate your existing ERP into the SMS. Emergency Response is a functional requirement of SMS.
